What should I bring to my physical?

Before you arrive at Dr. Lopez’s office for an immigration physical, make sure to bring with you the following items:

  • Form I-693
  • Vaccination records
  • Photo ID
  • Insurance card if you have one
  • Form of payment

Be prepared to talk about past surgeries, medical conditions, and any medications you take.

What happens during an immigration physical?

During an immigration physical, Dr. Lopez and her team check your weight, pulse, blood pressure, and other vital signs. She discusses your medical history and asks about any symptoms you may be experiencing.
